I agree to bring my cat/dog to the Valley SPCA la Vallée to be spayed at no cost to me. The cat/dog will be given flea treatment only if needed. 

You are responsible for the kittens/puppies until they are 8 weeks old. We will ask for a visit at 6 weeks old to have their first exam and for them to be given their first vaccine and treatments. The adult cat/dog will have to be kept in a place where she will not get pregnant again, if she gets pregnant again the owner will have to pay a fee of (70$) when we get her spayed. 

If all of the kittens/puppies die, we will NOT spay the mother.

In exchange, you will release all kittens/puppies from the litter, at the age of 8 weeks, to the Valley SPCA la Vallée. By checking the box, I am aware that this contract is final.
