If you are approved for this assistance program and do not have a vet, the Grand Falls Veterinary Clinic will still do the spay/neuter. 

You understand that for the time being, the Grand Falls Veterinary Clinic does not take any new clients and will take your cat for the spay/neuter ONLY.

I hereby certify that the foregoing is true and correct, and that I have not omitted anything which would make my application false or misleading.

Waver of Responsibility

The Valley SPCA la Vallée Inc. shall not be held responsible for any injury (including death) to my animal while participating in the Spay/Neuter  Program (SNIP). I hereby release and waive any right of action against the Valley SPCA la vallee, their heirs, assigns, servants, agents, employees, or Board of Directors for any such loss or injury (including death).

The Valley SPCA la Vallée is not responsible for any additional costs associated with the spay/neuter surgery. By checking this box, I acknowledge that I have read this Waiver of Responsibility and have voluntarily accepted it.
